Customer Information Form for Computer Repair

In order to complete your computer repair in the most thorough and efficient manner, certain information is required. Please fill out the customer information form completely, sign it, and include it with your equipment.

Failure to provide complete information can result in repair delays and/or additional expense.     

Please note that you have a choice of service levels. Standard service is performed in order received and typically completed within 5 business days. Urgent and high priority service is available (by prior arrangement) if you need it. If you can give me a break and allow me more felxibility to schedule work, I can give you a break on the price. You can save 20% by choosing economy service. This generally will take a few more days to complete since higher priority service will take precedence.

For fastest service you must bring the computer to my shop. Please call first to make arrangements.

I will pick up by arrangement at other locations, at no extra charge when it fits my schedule and is incidental to other travel. However, travel time charges will apply for any required special trip to pick up or deliver, whether at your business or home, or at any other location.

Please do not drop off any equipment at any location without contacting me first.

Use this form for on-site service, to describe your problem and provide necessary information. Having this form filled out ahead of time and available, along with all necessary software or other items, will save time and result in more economical service.
